• Present copies of passports of the contracting parties with occupations listed
• If widowed, present death certificate of the deceased

• If minors under 21 years of age, they must have authorization from the parents
• Two adult witnesses, if foreigners present copies of passport, occupation, civil status and must not be related to the contracting party
• Present medical report of contracting party with medical seal and signature
• The cost of the Marriage Certificate is included
• Present Single status from the consular, embassy, civil registry, or apposite seal and signed from his or her country or city
*Note: The Single status cannot be from a public notary. The Single status must be from the place you were registered.
